The pitch for data in healthcare has been consistent for a decade: collect more of it, and better decisions will follow. Patient portals, wearables, digital outcome measures, HRV tracking — the data available to practitioners has grown enormously. The quality of routine clinical decisions hasn't kept pace.
The problem isn't a shortage of data. It's a shortage of data that's structured for the moment when a decision actually needs to be made.
What a practitioner actually needs to know
Consider what a physiotherapist needs when they sit down with a client for a six-week post-ACL-reconstruction review:
They need to know the current functional capacity — not as a raw score, but relative to pre-injury baseline and age-matched norms. They need to know the trajectory: is the client improving, plateauing, or declining? They need to know what the exercise physiologist who saw the client last week documented — not buried in a note, but surfaced where it's relevant. They need the PSFS scores from intake and the most recent session, side by side, so they can see whether the client's subjective experience of function matches the objective measures.
What they usually have is: a PDF from the previous appointment, PSFS scores in a spreadsheet someone emailed them, and a verbal handoff from the EP in the corridor.
The data existed. It wasn't structured for the decision.
The data lake fallacy
A common response to clinical data problems is to consolidate everything into a central repository — the "data lake" model. All the data in one place, queryable, available.
This doesn't solve the problem. It relocates it.
A data lake is useful for retrospective analysis — research questions, population-level audit, QI initiatives. It's not useful for the clinician in the room who needs to know, right now, whether this client is ready to progress from single-leg press to single-leg squat.
The reason is that clinical decisions are contextual in a way that raw data can't account for. The question isn't "what is this client's leg press 1RM?" The question is "what is this client's leg press 1RM, relative to their other leg, relative to where they were four weeks ago, and given that they reported significant soreness after last Tuesday's session, should I push today or hold?"
Answering that question requires not just data, but data organised around a clinical decision framework. The signals need to be pre-filtered, contextualised, and surfaced at the right moment — before the practitioner walks into the room, not after they've spent fifteen minutes searching for it.
The difference between measurement and monitoring
There's a distinction worth drawing between measuring outcomes and monitoring them.
Measurement is point-in-time. You administer a PSFS at intake and at discharge. You have two data points. They're useful for demonstrating change, but they don't tell you much about the trajectory — when improvement stalled, whether a particular intervention correlated with acceleration, whether the rate of change suggests the client will hit functional targets before their return-to-sport date.
Monitoring is continuous. Every session contributes a data point. Pain ratings, load completed, subjective energy, specific outcome measure scores — all of these, logged consistently, create a picture that's actually useful for making decisions mid-treatment.
Most clinical software supports measurement. It gives you a form to fill in at intake and discharge. The session-by-session monitoring happens in practitioners' heads, if it happens at all.
This is the gap that costs clinics the most — not in dollars, but in clinical quality. Subtle deterioration patterns get missed. Recovery timelines get misjudged because nobody has a clear picture of the actual trajectory. Discharge decisions get made on vibes rather than data.
Curated signals, not comprehensive records
The solution isn't to show practitioners more data. It's to show them the right data, configured for the clinical decision they're making.
For a return-to-sport review, the right signals are: limb symmetry index on key tests, pain scores over the last four weeks, training load compliance, and any flags raised by the strength coach. Surface those — clearly, in context — and the practitioner can make a fast, informed decision.
For a chronic pain management appointment, the right signals are different: PSFS trend, DASS-21 scores, consistency of home exercise completion, the correlation between load and pain response over the past six weeks.
Building this kind of contextual signal surfacing requires a system that understands clinical workflows — not just a place to store data. It requires the programming and the clinical record to be integrated, so that what the AIMS engine logs during training sessions feeds directly into what the allied health practitioner sees at the next review. It requires outcome measures to be structured at intake, not as a one-off form but as ongoing monitoring instruments.
What this looks like in practice
